      摘要 目的:通过评价和比较传统手部肌力训练方法和计算机辅助训练方法对手功能恢复的作用,探讨计算机辅助肌力训练对断腕断掌再植患者手功能的影响。 方法:选择24例断腕断掌再植患者,随机分为治疗组和对照组,均接受常规物理因子、关节活动度、日常生活相关性治疗性活动等康复治疗。治疗组采用自主研制的计算机辅助装置进行握力、捏力训练,而对照组采用传统的方法(使用弹力治疗带、治疗泥等)进行训练,两组均为每次20min,一周5次,共4周。治疗前及治疗4周后进行评定,包括关节总主动活动度(TAM)、侧捏力、握力、日常生活活动(ADL)能力以及上肢功能指数(UEFI)评定。 结果:组内比较,两组患者治疗4周后TAM、侧捏力、握力、ADL评分和UEFI评分均较治疗前改善(P<0.05)。与对照组相比,治疗组侧捏力、握力、ADL、UEFI评分提高幅度更大,差异有显著意义(P<0.05)。而两组的TAM改善程度无显著差异(P>0.05)。 结论:计算辅助肌力训练能更好地增强断腕断掌再植患者的手部力量,更有效地提高手功能,是值得推广的一种治疗方法。

Abstract:
      Abstract Objective: To compare the effect of computer-assisted hand strength training with conventional strength training on hand function in patients who underwent replantation of amputation of hand at wrist/palmar level. Method: Twenty-four patients who underwent replantation of amputation of hand at wrist/palmar level were randomly assigned to an experimental group and control group. Participants in both groups received rehabilitation treatment including physical modalities, joint range of motion exercises, therapeutic activities,etc. Participants in the experimental group received pinch and grip strength training by computer-assisted device, while those in the control group were given strength training (using therapy putty and theraband), 20min per session, 5 times a week, totally 4 weeks for 2 groups. Outcome measurement including total active motion (TAM), grip and pinch strength, activities of daily living (ADL) and upper extremity function index (UEFI) were carried out at the beginning and the end of trial. Result: After training, significant improvements were found on all these measurements in the experimental group and control group(P<0.05). Participants receiving computer-assisted therapy had significant improvement in key pinch and grip strength,ADL and UEFI scores, compared to patients who underwent conventional therapy(P<0.05). No significant difference was found between two groups in TAM(P>0.05). Conclusion: Computer-assisted strength training is effective on improvement of hand strength and function. Therefore, it may be a promising treatment alternative for hand injuries.
