      摘要 目的:观察基于主动前庭康复训练联合限制-诱导运动疗法对痉挛型偏瘫儿童上肢功能及平衡功能的影响。 方法:选取2023年1月—2023年11月来中国医科大学附属盛京医院康复中心接受康复治疗的痉挛型偏瘫患儿40例为研究对象,采用随机数字表法分为对照组(n=20)与试验组(n=20)。对照组进行常规的上肢限制-诱导运动疗法方案,试验组在对照组的基础上给予基于主动的前庭康复训练,在治疗前和治疗12周后分别对两组患儿使用精细运动能力测试量表(FMFM)、改良Ashworth量表(MAS)及Berg平衡量表进行评估。 结果:经过12周的干预后,两组FMFM评分、MAS评分及Berg评分均有改善,试验组FMFM评分及Berg评分高于对照组,差异具有显著性意义(P<0.05),而MAS评分降低效果与对照组比较不明显,差异无显著性意义(P>0.05)。 结论:基于主动前庭康复训练联合限制-诱导运动疗法能提升儿童训练兴趣,改善痉挛型偏瘫儿童上肢肌肉张力,有效提高上肢精细运动功能及平衡功能,具有临床应用价值。

      Abstract Objective: To explore the effects of constraint-induced movement therapy combined with active vestibular rehabilitation training on upper limb function, muscle tone and balance function in children with spastic hemiplegia. Method: A total of 40 children with spastic hemiplegia in our hospital from January to November 2023 were selected, and they were divided into control group (n=20) and experimental group (n=20) by random number table method.The control group received conventional upper limb constraint-induced movement therapy,while the experimental group received conventional upper limb constraint-induced movement therapy and active vestibular rehabilitation training for 12 weeks. They were assessed with Fine Motor Function Measure(FMFM), Modified Ashworth Scale (MAS) and Berg Balance Scale before and after treatment. Result: After 12 weeks of intervention, the FMFM score, MAS score and Berg score were improved in the two groups, and the FMFM score and Berg score in the experimental group were higher than those in the control group with significant difference(P<0.05).There were no statistically significant difference in the reduction of MAS score grade when compared with the control group (P>0.05). Conclusion: Active vestibular rehabilitation training combined with constraint-induced movement therapy can enhance children's interest in training, reduce upper limb muscle tone, and effectively improve upper limb motor function and balance function in children with spastic hemiplegia,which has a certain clinical value.
