目的:本研究旨在通过项目反应理论(item response theory,IRT)对维持性血液透析(maintenance hemodialysis,MHD)患者的生存质量水平进行量化评估,通过参数化模型分析KDQOL-36量表条目,为改善MHD患者生存质量提供科学依据。
方法:用肾病生存质量量表(kidney disease quality of life instrument-36,KDQOL-36)对160例MHD患者进行多中心、横断面的生存质量评估。聚焦于躯体和心理维度,分别拟合并比较多种IRT模型,筛选出拟合优度最佳的模型及拟合良好的条目进行参数化建模,最后检验其心理测量特性。
结果:本研究最终对躯体、心理维度均建立了6项条目的二参数逻辑斯蒂模型(two-parameter logistic model,2PLM)。对于躯体维度,2PLM显示出最优模型拟合(比较拟合指数CFI=0.984)和良好信度(潜在类别信度系数LCRC=0.808);效度检验表明,潜在特质分数(θ)与生理健康评分(PCS)(P<0.01,r=0.45) 及肾病症状与问题领域(SPKD)(P<0.01,r=0.25)呈显著正相关。对于心理维度,2PLM同样显示出最优拟合(CFI=0.971)和良好信度(LCRC=0.789);效度检验显示,θ值与心理健康评分(MCS)(P<0.01,r=0.29) 及肾病负担领域(BKD)(P<0.01,r=0.33) 呈显著正相关。
结论:本研究应用IRT成功构建了针对KDQOL-36躯体维度和心理维度的二参数逻辑斯蒂模型,模型均显示出良好的心理测量学特性(信度和效度),为评估MHD患者生存质量水平提供了参数化工具,为肾病管理决策提供量化依据。 |

| Abstract
Objective: This study aimed to quantitatively assess the quality of life (QoL) in maintenance hemodialysis (MHD) patients using item response theory (IRT), and to analyze the items of the KDQOL-36 scale via parametric modeling, to support QoL improvement strategies in this population.
Method: A multicenter cross-sectional study was conducted among 160 MHD patients using the KDQOL-36 scale. Focusing on physical and mental health dimensions,optimal IRT models were selected to screen well-fitting items and establish parametric models. Psychometric properties of the final models were rigorously validated.
Result: For both the physical and mental dimensions, a 6-item two-parameter logistic model (2PLM) was ultimately established. For the physical dimension, the 2PLM demonstrated the best model fit (CFI=0.984) and good reliability (LCRC=0.808). Validity testing showed that the individual ability parameter (θ) had significant positive correlations with the physical component summary score (PCS) (P=2.49×10﹣35,r=0.45) and the symptoms and problems of kidney disease domain(SPKD) (P=1.74×10﹣12,r=0.25). For the mental dimension, the 2PLM also showed the best model fit(CFI=0.971) and good reliability(LCRC=0.789). Validity testing revealed significant positive correlations between θ and the mental component summary score(MCS) (P=1.91×10﹣5,r=0.29) and the burden of kidney disease domain(BKD) (P=2.43×10﹣4,r=0.33).
Conclusion: IRT-based modeling of the KDQOL-36 yielded robust two-parameter logistic models for both physical and mental dimensions, providing parametric tools for assessing the QoL levels of MHD patients and offering a quantitative basis for guiding kidney disease management. |
