目的:探讨上下肢联动训练对脑卒中偏瘫患者运动功能的效应。
方法:选取中山大学附属第七医院和深圳市罗湖区人民医院脑卒中偏瘫患者120例,参照随机数字表法将入组患者分为对照组和试验组,每组60例。对照组采用常规康复训练,试验组采用常规康复训练及上下肢联动训练。比较试验前后、3个月随访的组间Fugl-Meyer 评分(Fugl-Meyer assessment,FMA)、Berg 评分(Berg balance scale,BBS)、上肢动作研究量表 (action research arm test,ARAT),以及改良Barthel指数评估(modified Barthel index, MBI)变化。
结果:试验组干预后和随访3个月FMA下肢、FMA总分、ARAT、BBS、MBI与干预前相比显著提高(P<0.05);试验组与对照组之间的干预后与随访3个月的FMA下肢、FMA总分和ARAT均有显著差异(P<0.05)。
结论:相较于常规康复治疗方法,上下肢联动训练改善脑卒中患者的运动功能更明显,并拥有一定的延续效果,特别是改善上肢的功能。 |

| Abstract
Objective: To investigate the effectiveness of extremety-linkage training on motor function and balance function of poststroke patients with hemiplegia.
Method: Total 120 patients with stroke hemiplegia from The Seventh Affiliated Hospital of Sun Yat-sen University and Luohu People's Hospital of Shenzhen were divided into control group and experimental group by random number table method, with 60 cases in each group. The control group received conventional rehabilitation training, and the experimental group received conventional rehabilitation training and extremety-linkage training. The total training time of the two groups was the same. The changes of Fugl-Meyer assessment(FMA), Berg balance scale(BBS), action research arm test(ARAT) and modified Barthel index(MBI) were compared before and after treatment and at 3 months’ follow-up.
Result: Lower extremity and total score of FMA, ARAT and BBS of the experimental group were significantly increased after intervention and at 3 months’ follow-up compared with before intervention (P<0.05). There were significant differences in lower extremity and total score of FMA and ARAT between the experimental group and the control group after intervention and at 3 months’ follow-up (P<0.05).
Conclusion: Compared with conventional rehabilitation therapy, extremety-linkage training can improve the motor function of stroke patients more obviously, and has a certain continuation effect, especially to improve the function of the upper limb, which has important guiding value in clinical practice and can be used as a supplement to the conventional rehabilitation therapy. |
