目的:观察颅脑损伤术后患者再发脑肿胀的发生率及其对患者功能恢复的影响,加强对脑肿胀的预防和治疗。方法:收集2000年1月-2005年1月在我科住院的颅脑损伤术后患者310例,根据患者临床症状,头颅CT(或MRI)检查及临床治疗情况,确诊再发脑肿胀患者16例,计算再发脑肿胀的发生率:选择同期住院的颅脑损伤术后末发生脑肿胀患者并排除其他影响因素的病例150例作为对照组。比较两组患者的平均住院日和平均住院费用:评估两组患者康复治疗前后的Fugl-Meyer评分和Barthel指数。结果:颅脑损伤术后患者再发脑肿胀发生率为5.2%;再发脑肿胀组的患者平均住院日和平均住院费用均较对照组明显增加,两者间差异有显著性意义(P〈0.01);两组患者康复治疗前Fugl-Meyer评分和Barthel指数比较差异均无显著性意义(P〉0.05),出院时脑肿胀组Fugl-Meyer评分和Barthel指数均低于对照组,两者间差异有显著性意义(P〈0.05)。结论:颅脑损伤术后患者再发脑肿胀会不同程度影响患者的功能恢复.应及时发现和治疗,减少其对功能恢复的影响。 |

Objective: To study the incidence of recurrent brain edema in patients of brain injury after operation and its effects on limb function. Method:There were 310 inpatients of brain injury underwent operation in our department from Jan 2000 to Jan 2005. Among these inpatients, 16 cases were diagnosed as recurrent brain edema based on symptom and CTMRI. 150 inpatients without recurrent brain swelling were selected as control group. The average days of hoopitalization and average expenses in the hospital were compared between the two groups. All patients were assessed with Fugl-Meyer Assessment and Barthel Index before and after rehabilitation treatment.Result:The incidence of recurrent brain swelling in patients of brain injury following operation was 5.2%. In the brain swelling group, the average days and average expenses in hospital were significant higher than the control group(P<0.01). There were no significant different on the score of Fugl-Meyer Assessment and Barthel Index between the two groups before rehabilitation treatment(P>0.05). The score of Fugl-Meyer Assessment and Barthel Index in the brain edema group were significant lower than the control group(P<0.05) on discharge. Conclusion: The present of recurrent brain swelling in patients of brain injury after operation would affect their function recovery. |
