摘要:
      目的:探讨轻度认知障碍(MCI)患者血浆同型半胱氨酸(Hcy)水平,并观察叶酸、VitB12对高同型半胱氨酸血症的干预作用。方法:应用荧光偏振分析法测定80例MCI患者(MCI组)血浆Hcy、叶酸、VitB12水平,并与78例健康人(对照组)对照;其中MCI患者中选出高Hcy血症者47例再按有无接受叶酸、VitB12治疗随机分为治疗组和非治疗组,观察治疗前后Hcy水平变化及MMSE评分变化。结果:MCI组血浆Hcy水平较对照组高(P<0.05);叶酸、VitB12水平较对照组低(P<0.05);叶酸、VitB12治疗组Hcy水平较治疗前下降(P<0.05);治疗后治疗组Hcy水平低于非治疗组(P<0.05)。治疗组简易智能状态量表(MMSE)评分较治疗前升高,但差异无显著性(P>0.05), 治疗后治疗组MMSE评分与非治疗组相比差异无显著性(P>0.05)。结论:MCI患者存在高Hcy血症及叶酸和VitB12的缺乏;补充叶酸和VitB12可降低血浆Hcy水平。

Abstract:
      Objective:To explore the level of plasma homocysteine(Hcy) in patients with mild cognitive impairment(MCI) and to observe the intervention role of folic acid and VitB12 to hyperhomocysteinemia. Method:Fluorescence polarization analyse and microparticle enzyme immunoassay were respectively applied to determine plasma Hcy level and plasma folic acid and VitB12 levels in 80 patients with MCI(MCI group), 78 healthy people were as controls (control group). Forty-seven cases with hyperhomocysteinemia were choosed from MCI group and randomly divided into treatment group and non-treatment group according to whether they had received folic acid and VitB12 treatment or not. The level of plasma Hcy and mini-mental state examination(MMSE) score were observed before and after treatment. Result: In MCI group plasma Hcy level was higher than that in control group(P<0.05), and plasma folic acid and VitB12 levels were lower than that in control group(P<0.05). After treatment with folic acid and Vit B12 plasma Hcy level in treatment group decreased compared with before treatment(P<0.05) and was lower than non-treatment group(P<0.05). After treatment MMSE scores in treatment group increased than that before treatment, but there was no significant difference(P>0.05). After treatment there was no significant difference of MMSE scores between treatment group and non-treatment group(P>0.05). Conclusion: In MCI patients especially in hyperhonocysteinemia level of plasma Hcy is higher and levels of folic acid and VitB12 are lower; level of plasma Hcy could be reduced by adding folic acid and VitB12.
