      摘要 目的:探究有氧舞蹈对于轻度认知障碍(mild cognitive impairment, MCI)患者认知功能和脑白质纤维束完整性的干预作用。 方法:入组38例MCI患者,随机分成两组:运动组(n=19)和对照组(n=19)。运动组接受35min/次、3次/周、为期3个月的有氧舞蹈干预;对照组仅接受简单的健康教育。所有受试者在干预前、干预3个月后均接受认知功能评估及头颅弥散张量成像(diffusion tensor imaging, DTl)检查。 结果:3个月有氧舞蹈干预后,运动组扣带束双侧扣带回、扣带束左侧海马、左侧上纵束等白质纤维束的各向异性分数(fractional anisotropy, FA)值与干预前比较显著性升高(P<0.05),而对照组3个月后未发现FA值升高;运动组干预后较干预前简易精神状态评分(mini-mental state examination, MMSE)、蒙特利尔认知评估(Montreal cognitive assessment, MoCA)评分、韦氏逻辑记忆(Wechsler memory scale-revised, WMS-R)测试评分均有显著性差异(P<0.05);与对照组比较,运动组干预前后韦氏逻辑记忆评分的改善存在显著性差异(P<0.05)。 结论:有氧舞蹈能够改善MCI患者扣带束双侧扣带回、扣带束左侧海马、左侧上纵束等白质纤维束的FA值和认知功能,DTI成像为评估有氧训练对MCI患者干预作用的研究提供了影像学定量评估方法。

Abstract:
      Abstract Objective: To explore effects of aerobic dance on cognition and white matter fiber tracts integrity in patients with mild cognitive impairment. Method: A total of 38 subjects with mild cognitive impairment(MCI) were randomized into two groups: the treatment group (19 cases) and the control group (19 cases). The treatment group accepted aerobic dance for 35 minutes a session, 3 times a week for a total of 3 months; the control group only received health education. All the subjects completed cognitive assessments and diffusion tensor imaging (DTl) sequence of MR at baseline and 3 months after intervention. Result: The fractional anisotropy (FA) values were significantly increased in the white matter fiber bundles (such as the bilateral cingulate gyrus of the cingulate tract, the left hippocampus of the cingulate tract, the left superior longitudinal tract) in treatment group after 3 months’ aerobic dance (P<0.05).There were no significant differences between the FA values of the control group before and after intervention. The scores of mini-mental state examination (MMSE), Montreal cognitive assessment (MoCA), Wechsler memory scale-revised version (WMS-R) in treatment group were significantly improved after 3 months’ aerobic dance when compared with the baseline (P<0.05). The WMS-R of treatment group improved significantly after 3 months' intervention when compared with the control group (P<0.05). Conclusion: Aerobic dance improves the FA value in the white matter fiber bundles including the bilateral cingulate gyrus of the cingulate tract, the left hippocampus of the cingulate tract, the left superior longitudinal tract and the cognitive function in patients with MCI. DTI sequence of MR provides a quantitative method for evaluating the effect of aerobic exercise.
