      摘要 目的:研究胫神经电刺激对脊髓损伤大鼠神经源性膀胱及膀胱中辣椒素受体(transient receptor potentia vanilloid 1,TRPV1)通道蛋白表达的变化,以期探究其可能存在的机制。 方法:将24只SD大鼠随机分成四组,每组6只:正常组,假手术组,脊髓损伤组,干预组。对于脊髓损伤组及干预组,使用改良Allen’s打击法进行脊髓损伤造模。同时在2周后对干预组进行电刺激,每日2次,每次20min,每周5天,持续4周;实验第6周以尿流动力学指标及膀胱组织HE染色衡量干预效果,使用实时定量PCR及Western Blot方法检测膀胱中TRPV1通道蛋白的表达。 结果:干预组膀胱顺应性为0.05±0.01(ml/cmH2O),脊髓损伤组大鼠膀胱顺应性为0.03±0.01(ml/cmH2O),干预组较脊髓损伤组相比膀胱低顺应状态明显改善(P<0.05),膀胱组织炎性细胞浸润明显减轻,与此同时干预组TRPV1通道蛋白在转录及翻译水平均明显升高(P<0.05)。 结论:胫神经电刺激有效促进了脊髓损伤大鼠膀胱中TRPV1通道蛋白的表达,并对于神经源性膀胱功能恢复起到了明显的促进作用。

Abstract:
      Abstract Objective:To study the changes of posterior tibial nerve stimulation on neurogenic bladder and TRPV1 expression in rats with spinal cord injury and to explore its possible mechanism. Method:Twenty four SD rats were randomly divided into four groups with 6 rats in each group including normal group, sham operation group, spinal cord injury group(SCI group) and intervention group. Spinal cord injury model was established by modified Allen’s method and posterior tibial nerve stimulation was performed in SCI group at 2 weeks after modeling (20 minutes twice a day, 5 days a week, lasting 4 weeks). Urodynamic indexes and HE staining of bladder tissue were used to evaluate the therapeutic effect at 6 weeks after modeling. The expression of TRPV1 channel protein in bladder was detected by real-time PCR and Western Blot. Result:The bladder compliance in intervention group was 0.05±0.01(ml/cmH2O),while 0.03±0.01(ml/cmH2O) in SCI group. It suggested that the low compliance of bladder in intervention group significantly improved compared with SCI group(P<0.05).And the infiltration of inflammatory cells in bladder tissue was significantly reduced. At the same time, the level of TRPV1 channel protein in the treatment group was significantly higher than that in the control group(P<0.05). Conclusion:Posterior tibial nerve stimulation can effectively promote the expression of TRPV1 channel protein in bladder of rats with spinal cord injury, and play a significant role in promoting the recovery of neurogenic bladder function.
