      摘要 目的:观察发散式体外冲击波治疗对轻、中度腕管综合征患者疼痛、手功能和抑郁的临床疗效。 方法:将纳入标准的60例腕管综合征患者随机分为试验组(n=30)和对照组(n=30)。两组都进行常规药物治疗和健康教育,试验组在对照组的基础上给予每周2次发散式体外冲击波治疗。于治疗前、治疗6周后对两组患者进行评估。评估指标采用视觉模拟量表(VAS)、波士顿腕管综合征问卷(BCTQ)、正中神经感觉神经传导(SNCV)、正中神经末端运动潜伏期(DML)及贝克抑郁量表(BDI)。 结果:两组治疗前各项评估指标组间差异无显著性意义(P>0.05);治疗6周后,两组患者SNCV速度较治疗前明显提高(P<0.05),且试验组高于对照组(P<0.05);VAS、BCTQ、DML和BDI指标较治疗前明显降低(P<0.05),且试验组低于对照组(P<0.05)。 结论:发散式体外冲击波治疗能够缓解轻、中度腕管综合征(CTS)患者的疼痛,提高手功能,对患者的抑郁状态产生积极影响。

      Abstract Objective:To observe the clinical effect of radial extracorporeal shock wave on pain, hand function and depression in patients with mild to moderate carpal tunnel syndrome. Method:Sixty carpal tunnel syndrome patients who met the inclusion criteria were divided into the experimental group (n=30) and the control group (n=30).Both groups received routine drug therapy and health education.On the basis of the control group, the experimental group was given radial extracorporeal shock wave therapy twice a week. The evaluation of two groups were performed before and at the six weeks over the treatment. Visual analogue scale (VAS), Boston carpal tunnel syndrome questionnaire (BCTQ), median sensory nerve conduction (SNCV), distal motor latency(DML)and baker depression scale (BDI) were used for evaluation. Result:There was no significant difference between the two groups in each evaluation index before treatment (P>0.05). After 6 weeks of treatment, SNCV score of patients in both groups was significantly increased compared with that before treatment (P<0.05), and the experimental group was higher than the control group (P<0.05). VAS、BCTQ、DML and BDI scores were significantly lower than those before treatment (P>0.05), and the experimental group was lower than the control group (P<0.05). Conclusion:Radial extracorporeal shock wave therapy can alleviate the pain of mild to moderate CTS patients, improve hand function, and have a positive impact on the depressive state of patients.
