摘要:
      摘要 目的:观察下肢机器人训练联合呼吸训练对脑卒中患者肺功能及运动耐力的影响。 方法:将60例脑卒中患者随机分为机器人组(R组)、呼吸组(RT组)和联合组(UT组),每组各20例。3组均给予常规的药物治疗及康复训练(30min/次,1次/d,5d/周,共4周),R组在常规康复训练的基础上增加下肢机器人训练(1次/d,40min/次,5d/周,共4周),RT组在常规康复训练的基础上增加呼吸训练(1次/d,40min/次,5d/周,共4周),UT组在常规康复训练的基础上同时进行下肢机器人训练和呼吸训练(均1次/d,均20min/次,5d/周,共4周)。分别于治疗前和治疗4周后对患者进行评估,采用力肺活量(FVC)、第1秒用力呼气容积(FEV1)、峰值呼气流速(PEF)和每分钟最大通气量(MVV)评估患者的肺功能,峰值摄氧量(VO2peak)、无氧阈值(AT)及6min步行试验(6MWT)评估患者的运动耐力。 结果:治疗前,3组患者的FVC、FEV1、PEF、MVV、VO2peak、AT及6MWT评分均无明显差异(P>0.05);治疗4周后,3组患者的FVC、FEV1、PEF、MVV、VO2peak、AT及6MWT均较治疗前明显改善(P<0.05);且UT组的各项评分均较R组和RT组改善更明显(P<0.05);且RT组较R组的FVC、FEV1、PEF、MVV、VO2peak、AT的评分也有明显提高(P<0.05),但6MWT评分比较无显著性差异(P>0.05)。 结论:在常规康复训练的基础上,呼吸训练和下肢机器人训练均能改善脑卒中患者肺功能及运动耐力,两者联合治疗效果更加显著。

Abstract:
      Abstract Objective:To observe the effect of lower limb robot training combined with respiratory training on pulmonary function and exercise endurance of stroke patients. Method:Sixty stroke patients were randomly divided into robot group (R group), respiratory training group (RT group) and united training group (UT group), 20 cases in each group. Three groups were given conventional drug treatment and rehabilitation training for 30min, once a day, 5 days a week for 4 weeks. R group added lower limb robot training on the basis of conventional rehabilitation training for 40min, once a day, 5 days a week for 4 weeks, RT group added respiratory training on the basis of conventional rehabilitation training for 40min, once a day, 5 days a week for 4 weeks, UT group added the lower limb robot training and respiratory training on the basis of conventional rehabilitation training for 20 minutes a time, once a day, 5 days a week for 4 weeks. FVC, FEV1, PEF and MVV were used to evaluate the pulmonary function, VO2peak, AT and 6MWT were used to evaluate the exercise endurance. Result:There was no significant difference in FVC, FEV1, PEF, MVV, VO2peak, AT and 6MWT scores among the three groups before treatment(P>0.05); after 4 weeks of treatment, FVC, FEV1, PEF, MVV, VO2peak, AT and 6MWT scores of the three groups were significantly higher than those before treatment(P<0.05); and the scores of UT group were significantly more improved compared with R group and RT group(P<0.05); and the scores of FVC,FEV1,PEF,MVV,VO2peak and AT in RT group were also significantly more improved compared with R group(P<0.05),but there was no significant difference in 6MWT score(P>0.05). Conclusion: On the basis of conventional rehabilitation training, respiratory training and lower limb robot training can improve the pulmonary function and exercise endurance of stroke patients, and the combined treatment effect is more significant.
